Report Harm 

Report a Side Effect to the Irish Health Products Regulatory Authority (HPRA)

Who Can Report: Patients, family members and carers can report suspected drug side effects directly to the HPRA - they do not have to wait for a doctor to do it for them.

What Can Be Reported: Any suspected side effect, including long-term withdrawal effects, severe reactions, or symptoms linked to BIND (Benzodiazapine-Induced Neurological Dysfunction)

Information Needed by The HPRA: 

• Details of the medicine (brand name, dosage, and batch number if known)

• A clear description of the symptoms or side effects experienced.

• Background infromation (such as other medications taken or underlying conditions)
